FAO Cost Center Change Request Form

FY23 FAO Cost Center Change Request Form is now available to use and applied to all FAO Types excluding GR5s.

Completed form needs to be submitted to your Division Finance Office.

The Divisional Finance Office will review and approve all requests and email the form to GeneralAccounting@UR.Rochester.edu BY JUNE 15th deadline.

Additional back up documentation requirement and instruction are provided as a reference.
