COVID-19 Vaccination Exemption Process

Reasonable accommodations may be available for individuals who are granted an exemption from vaccination due to religious or medical reasons, as outlined in the University’s COVID-19 Vaccine Policy (#165) and COVID-19 Healthcare Provider Vaccination Policy (#164). Employees who wish to receive an exemption must submit an exemption request form by following the process below.

Information about protocols for employees with an approved exemption can be found at the University’s COVID-19 Resource Center. Exemptions requested based on personal preference will not be approved.

EP1
  • Login to the website
  • Select the “Domain”. “UR” for those with UR credentials, and “URMC-SH’ for everyone else.
  • Enter the same username and password you use to login to your computer. This is NOT the same as your NetID.  This is also sometimes referred to as your “Active Directory” or “AD” credentials.

  • Select “I do not want to receive the COVID vaccine at this time.”
  • Select which exemption you want to request – Medical or Religious. The exemption request is also possible if you specify that you “have received at least one dose of a 2-dose vaccine, but do not plan on getting a second dose.
  • Specify your supervisors by starting to type their name in the search box. Supervisors will NOT receive your exemption request documentation.
  • Press the “Continue” button after selecting one or more supervisors.

  • You will be notified that your submission was received, but that you also “must upload at least one supporting document for this response”.
  • At the bottom of the page, you will be given a link to the required exemption form if you don’t already have it.  You will need to have it filled out, signed, and uploaded along with any additional supporting documentation.
  • Click the “Choose” button and select the document(s) you want to upload from your computer/phone.  You may click “Choose” again and select additional documents.  Once done selecting one or more documents, then click the “Upload” button.
EP9
  • You will briefly be shown a green “Success” popup indicating that the file(s) were successfully uploaded.
  • That completes your exemption request.
  • You will be able to change you selection of documents (add or remove files) or your selected vaccination status until your exemption request is In Process of being reviewed.
  • Once a decision is made on either Accepting or Denying your exemption request, you will be notified of the decision.
