Scholar Address Reporting Form

If you’ve moved or plan on moving, you must notify ISO within 10 days of the change. This applies to your US home address, mailing address, overseas home address (if it’s permanent) or temporary address changes, like extended vacations or work trips.

This form only updates our ISO records. You might also need to update your address with Payroll, Benefits, or other University offices.

How to update

J-1 scholars: Use this form below to update your address with ISO. We’ll then be prompted to update your address in your SEVIS record.
F-1 OPT employees (non-University sponsored like Fulbright, LASPAU, etc.): Use this form below to update your address with ISO. You must also contact your F-1 sponsor to have your address updated in your SEVIS record.
H-1B, TN, and O-1 employees: You must contact USCIS (US Citizenship and Immigration Service) directly on the USCIS website.
